Mega Evo Glitch
This doesn't always seem to happen, but if you mega evolve in a previous battle, your pokemon may already be in its mega state upon entering your next battle. In this case, I even removed the mega stone before battling.

If you find a pattern on this (on which kind of battles, where...) please tell me, so it's easier for me to trace the bug

I'll keep an eye on it, but after that battle, my Electruxo is still in its Mega form

I've found that the glitch only happens when I Mega Evolve while rebattling a trainer. After fighting a wild pokemon my Electruxo returns to its regular form. Additionally, when I Mega Evolve, it would appear as though his mega ability doesn't take over. At first I was unsure about the ability bit because it would never start raining like it should since Electruxo gets Drizzle, but I was hitting my Thunders anyways. I missed a Thunder after I Mega'd recently, so I made sure the stats changed as well (which they did).
TL;DR Mega Evo when rebattling causes pokemon to retain its form until battling a wild pokemon. Abilities for Megas don't work in the battle that they Mega'd, but do in next battle. Stats do change properly when they Mega.

I hadn't been paying much attention to this since the recent patches but today I noticed that, while the glitch with pokemon staying in their mega form outside of battle is fixed, the glitch with their ability not taking over upon evolving in battle is still there. For example, Electruxo's Drizzle doesn't cause rain to occur and Thunder will miss on occasion.

I can't be certain that it happesn for all pokemon. It's easy to tell for Electruxo because Drizzle causes rain to appear and that means that Thunder should have 100% accuracy. For Mega S51-A it would be harder to tell because Mega Launcher gives extra power to pulse moves, which would be harder to notice.

That part of the glitch appears to have been fixed. If you check the time stamps, I made this post at the release of 4.2.0
